Quinte Magnetic resonance imaging Essay Brenton-Cooper Medical Centre (BCMC) has outsourced its MRI operations to Quinte MRI, a seasoned and highly recognized MRI service provider. Unfortunately, after six weeks of operations Quinte MRI’s leased MRI machine is not meeting its expected outputs as projected and is causing concern to both Quinte MRI and BCMC which has begun to lose revenue via referrals away from its clinic. Further, BCMC’s reputation is now at risk which could result in additional loses to the centre. The root cause of the problem appears to lie with the scheduling of the scanning operations. Dr. Syed Haider, the owner of Quinte MRI, has tasked his business development coordinators with finding a solution to this problem and to report back within 2-days. Acting as David Wright and Kevin Saskiw, the business development coordinators, my solution to these issues are to regain control of the scheduling process as this is an integral part of the operations. Streamline the scheduling process to reduce variability and improve reliability. Introduce the use of technology to remove manual processes, improve process efficiencies, minimise errors and improve communication across the business lines of operation. I would also hire a new employee to support the scanning operations as there is no backup or adequately trained support for the only Technologist running the operations. Issues Identification Brenton-Cooper Medical Centre (BCMC) has outsourced its MRI operations to Quinte MRI, a seasoned and highly recognized MRI service provider. Unfortunately, after six weeks of operations Quinte MRI has not lived up to expectations and is not fulfilling its contractual obligations. Quinte MRI’s leased MRI machine is not meeting its expected outputs as projected and is causing concern to both Quinte MRI and BCMC which has begun to lose revenue via referrals away from its clinic. Further, BCMC’s reputation is now at risk which could result in additional loses to the centre. Quinte MRI is well aware of this as it stands to lose in both areas as well. If the firm cannot meets its contractual obligations and have its leased machine produce its expected output the loss of revenue would surely spell business failure and a subsequent loss of reputation. As well, the firm would probably be sued for breach of contract. On another level Quinte MRI has also identified potential issues with a stressed out, overworked employee whom is critical to the success of their business. Potentially, this employee could actually be one of the reasons of the problem that the firm is experiencing. He has identified issues surrounding the scheduling of patients for scanning and has indicated that the process needs to be fixed as it is not working. He further went on to indicate that the expectations from the radiologist for speedy delivery cannot be met due to the time it takes for him to process the patients’ MRI films. Environmental and Root Cause Analysis In my opinion there are several operational issues causing problems in the MRI scanning process. First, I believe that there is a problem with the scheduling of patients in that it lacks accuracy, consistency and clarity. Since this operation is being handled by BCMC, Quinte MRI is finding itself at a disadvantage in terms of being able to control this part of the operation. The operation appears to be purely manual, handled by several persons and is prone to input and interpretive errors. Since patients can be sent for scanning via two means (scheduled and same day(unscheduled)) some amount of variability will result. It is therefore imperative that the scheduling function be properly controlled and managed to reduce variability to minimum possible levels. Variability and uncertainty in the scheduling operation is negatively impacting capacity utilization resulting in an overall reduction of efficiency in the scanning process. Quinte MRI must balance the flow and increase capacity for greater efficiencies and to remain competitive. The bottleneck of the scanning process is the MRI machine and the time it takes to do each scan. Each scan may have different times associated with it depending on the type to be performed, limiting the capacity of the overall process. Each step in the scanning process is dependent upon the previous one therefore improvements need to start at the beginning. The objective here is to improve the process flow up to the point that the actual scan will take place. I also believe that patients are not being properly screened prior to arrival which is causing Quinte MRI losses in revenue and time. If a patient turns up and has to be turned away, or rescheduled for misdiagnosis there is a resulting disruption in the flow of patients which will impact the schedule and process and ultimately the pocket and reputation of the company. Further, it appears that the technologist is engaged in performing pre-screening services and this is a highly paid employee who should not be pre-screening patients. This tasked could best be left to a lower paid trained staff. From an operational perspective it appears that the initial implementation process of the new machine had a learning curve. This resulted in longer lead times for processing patients during the first few weeks until Jeff had found a rhythm. It appears that Jeff was either not properly trained or did not have sufficient experience in the use of that model machine. Communication, and barriers to, seems to be a fundamental problem in the whole scanning process. People and processes are not talking to each other in an efficient and effective manner. The patients are unsure of the process and what is expected of them resulting in missed dates, tardiness, improper attire, rescheduling, lost revenue, lack of confidence, etc. Responsibilities appear not to be clearly defined, or assigned, leading to inefficiencies in the process. As well, expectations of persons are not clearly defined and communicated ahead of time leading to missed schedules, frustrations and delays. Alternative and Options In my opinion Quinte MRI could request the transfer of the responsibilities of the scheduling process over to their firm. This could benefit the firm in that they would now have ultimate control of the scheduling process from start to finish. As it is the firm has to rely on BCMC to manage this function and this is causing many issues. I believe that an MR Technician could be hired to perform the scheduling tasks and also provide support to Jeff Sinclair. Quinte MRI is already paying for these services but not receiving value. Revenue loss from turn-aways alone (1.2 p/day) could adequately cover the costs of hiring someone to provide these services. Additional benefit would be backup support for Jeff during vacation or other away days. Training would be provided on the job to support Jeff and add additional new skills to the technician. Screening could be done well ahead of time before the patient arrives because someone with the knowledge and expertise is handling this. Quinte MRI could invest some capital in technology to support the scheduling and communication functions. The technology would be able to support scheduling combinations for more efficient processing of patients in the scanning process. Many persons (from BCMC and Quinte MRI) could have visibility into the system with relevant authority to administer or make changes. This technology would be able to eliminate some amount of confusion and errors based on interpretation as is currently happening. Savings from a smooth flowing process with no disruptions would easily cover upfront costs as well as any operating costs.

Raising Awareness for Health Equality The District health unit and Sudbury SDHU has established numerous plans/ initiative to enlighten and raise alertness towards health equality. The video Lets Start a Conversation about Health†¦ and Not Talk about Health Care at All. This video diverts attention from what society perceives health care problems are- doctor scarcity, hospital wait times, nurse scarcity, health care expenses/ cost- and prominences a new point of view. Awareness is drawn towards public health, fostering healthy demeanors, sustaining healthy environment and advocating for approaches to make health choice a simple one (Health Equality, n.d). In general the aim is to emphasize on equity and health prevention and this is achieved using analysis from the collective determinants of health indicated in the video and also from text book Countless findings check listed in the video aligns with textbook. There are numerous health issues that our community is accosted with like: diabetes, cancer, asthma, heart disease, obesity and many more, countless of us are in poor health (Health Equality, n.d). We are   educated that if we maintain healthy diet, a dynamic lifestyle, stop smoking as well as decrease excess drinking-we can stop ourselves from getting sick ( Health Equality, n.d ). There are many other elements that cause ill health that we mostly do not consider. There is a strong connection between an individuals socioeconomic status and their health. Recent statistics reveal the higher your income levels the less you will get ill and the video does a substantial job in bringing that view forward. When you receive less income you are usually more stressed.   Stressed about accommodation payment, family needs, transport, food and the checklist can go on. Stress is one of the principal cause of sickness. On an individual level there are numerous forces that influence ones health- activity, family, food, health care access, coping etc. health is the outcome of the lifestyle choice we make, the residence we can afford   and the social aggravation we face. These elements are internally connected to the education we acquire, our gross pay, and employment we hold and our perception of community. And these elements are also impacted by external elements such as culture, public strategies, and economy (Health Equality, n.d). This is connected to what we have been taught in class.   The video does a great job in communicating these health problems but they fail to be comprehensive with each factor stated. The video examines health income through a materialists view. This is the viewpoint that stress that social determinants of healths end results and how they are involved with social living conditions. The video does not indicate anything about the social comparison theories that are in the textbook. This is where   persons contrast their living condition to others   and if they sense they are wanting in any way they   encounter levels of stress, envy that impact their health ( Raphael .D, n.d ) . The video indicates that the preferable your socioeconomic status the healthier you are and this is accurate. However, there are also conditions where individual have everything at their disposal to be healthy and earn a good income yet have poor health. This can be induced by poor lifestyles choices, genetics but can be linked with the social collation theories. In the situation of Nadia from the video she was deeply suffering from asthma and got taken to the E.R again, she got taken to the E.R as a result of her smoking again, she began smoking again because she was stressed out about her accommodation molding, and she cant manage to fix it because the company she is working for made some cutbacks and she was affected. Nadia is not educated enough to do anything else. This is a chain response. The ending in the video concerning what Nadia would require for her health were that she requires low-cost housing and food, the power to make healthy   choices and efficient   coping skills. The video additionally explains how she would require healthy lifestyle modifications, education to get a better employment, enough money for clothing and food, good work benefits and a secure community with good family and friends. Also, what the video did not mention that is in the textbook regarding Nadias case is that she could encounter race, gender and ethnicity inequalities. Perhaps even with the good education she got, she may not be able to have immense income because of income inequalities as a result of ethnicity and race. Which will be a worse situation because she will have to repay her educational loan and take her maintenance expenses from that income (Raphael.D, n.d). Or maybe examine this through another point of view and view the negative style in workplaces at the moment. The individuals making the higher incomes tend to overwork themselves, prompting stress as well and even if Nadia acquires a good education and better paying job she too might get overburdened and overstrained by the task responsibility (Raphael.D, n.d). The average person watching the video would get it because the video does an excellent job in looking at the bigger idea of public policy issue. However, they would ignore that a video under the health equality resources do not mention   anything regarding race, income and ethnic inequality neither does it cover individuals with high incomes overworking themselves   causing elevated stress level as well. The information the video presents is concise and to the aim thus causing audience to feel like no information is lacking from the truth presented.

Literary Criticism of Fahrenheit 451 Don’t worry, be happy, or at least that’s what everyone in Ray Bradbury’s book Fahrenheit 451 thought. No matter what was going on around them, war, crime, or death, they were always happy†¦ Or were they? Ray Bradbury wrote books about censorship in society forming around being censored totally or partially from books and television. In Fahrenheit 451 the main character, Montag, is a fireman whose job it is to burn books to keep the public from reading then and coming up with their own thoughts and ideas and not the ideas that the government puts in their heads. Wile he is burning books one day he opens one to read it and becomes obsessed with reading books. He turns on his fire chief and burns him, and goes to live with people who also read books and memorize them so that they can be reprinted then society is ready for them again. Three people that show that they are happy on the outside but are not truly happy are Montag, Mildred and Mrs. Phelps.   Ã‚  Ã‚  Ã‚  Ã‚  In the story, Montag, who is a fireman for the city thinks he is happy until he meets a neighbor girl named Clarisse. Clarisse is considered an oddball in the society of Fahrenheit 451 because she is â€Å"antisocial† in their world which actually means that she is social by today’s standards. She talks to people about things instead of the people of 451 who talk about meaningless things that have no substance. Peter Sesario says that’s she is considered to be this way because, â€Å"She was a sensitive, observant person who questioned society and was consequently eliminated by the government (3).†One day as Montag and Clarisse are walking down the street Clarisse says to Montag, â€Å"Are you Happy?†(10). Montag is speechless and before he could come up with and answer Clarisse is gone. As he enters his house he talks to himself, â€Å"Of course I’m happy. What does she think? I’m not?†(10) When he is saying this he looks up to the ceiling where he has Hidden books which are things that actually make him happy instead of the things around him which he thinks makes him happy. She also notices that when Montag laughs it is a fake laugh and that he doesn’t really mean it when he laughs. Also when he smiles it is not a real smile which further goes to show that he isn’t happy.   Ã‚  Ã‚  Ã‚  Ã‚  Another character that seems like she is happy but really is n... ...racter other than Montag who sees for herself that she is not actually happy. Three of the main characters in this story who you may think are happy in the beginning of the book but you figure out how unhappy they really are by reading into the book are Montag, Mildred, and Mrs. Phelps. Montag is one of the few people in the book who actually realizes that he is not happy towards the end of the novel. Mildred never realizes what happiness really is and ends up leaving Montag because of her inability to grasp what happiness is really about. Mrs. Phelps, who finally realizes that she is not happy, gets mad at Montag when he shows her that she is not happy and does not appreciate the good that he has done in her life by showing her that she is a very heartless person. The Fall of the Berlin Wall essays Following the war in 1945, the United States of America, Britain, France, and the Soviet Union divided the city of Berlin into sectors. Each sector was under the control of a different country. The United States, Britain, and France joined their sections together to form a democratic state on May 24, 1949. This state was called the Federal Republic of Germany or West Germany. The Russian or Soviet part became a communist state. This state was called The German Democratic Republic or East Germany, formed on March 19th, 1949. This division between East Germany, West Germany, and across all Europe is what Winston Churchill classified as the "Iron Curtain" that was falling across Europe. The city of Berlin was deep within the Soviet-controlled area. Because Berlin was the capital city, in Russia's sector, the Allies (United States, Britain, and France) believed that this gave the Soviets an unfair advantage. Part of the agreement between the former allies was that Berlin was to be divided into four sectors. The Soviet part was called East Berlin and was the capital city of East Germany. The fact that the Allied part of Berlin was completely surrounded by Soviet land would become an increasingly difficult situation. There was a ten-week period while Berlin was solely under the Soviet control. The Allies needed time to gather the resources to start governing Berlin. During this time Russia removed much machinery from Berlin and transported it to Russia, which gave them the upper hand and allowed them to cripple the allies and their economy, and not have them have the upper hand using their own machinery. Life is a Gift Essay Life is a gift. It is an honor; a spark; an excitement. We all have a world of our own. A wise author, Albert Camus, once stated, â€Å"you will never be happy if you continue to search for what happiness consists of. You will never live if you are looking for the meaning of life† (Albert Camus). Life is about living to your fullest abilities; why waste our time looking for the meaning of each breath we take? Each person is a part of life. Each life has a world of its own. I believe that the meaning of life, defined by Victor E. Frankl, is to find meaning in our own life. Each and every person on this planet has a meaning to his own life, and is a unique and independent individual. And so, the question comes along, â€Å"Who am I? † The answer is I. I am who I am, and not one person can ever change that. My life is exceedingly meaningful, and is made up of a series of rituals, experiences, a plentiful amount of mistakes, and many various emotions. According to the article, The Rite of Passage, a ritual â€Å"help[s] us understand the changes in our lives and give meaning to experiences† we undergo. Rituals express who we are today, as well as the sum total of our actions, feelings, and thoughts that makes each person in this world, a single, individual person. An example of a major ritual and life-changing event in the Odyssey is the trial of the bow. During this scene, a disguised Odysseus surpassed a difficult task, as he without effort strung the challenging bow. Odysseus, after laying the arrow on the arch, shot the mighty bow â€Å"with careful aim, not miss[ing] an axe’s ring from first to last† (Homer 2011). This scene makes a change in not only Odysseus’s life, but also his son, Telemachus, and his wife, Penelope. The trial of the bow is an important Rite of Passage, offering significance to each one of their lives. Just like Odysseus, I have progressed through a numerous amount of rituals, along with experiences, mistakes, truths and actions, all leading up to the fourteen year old girl I am today. Although each person has a different opinion on what surrounds us, I believe the nature of the universe is full of the unknown. The world is a scary place to be in, it encloses us in, with nothing but mystery. It is a frightening thing, to move on with your life not knowing what will happen in the next few moments, but that is what life is all about; the unknown. We surround ourselves with questions that can never be answered; yet we need to learn to live life in the present, not thinking about the future with every step we take. No one in this world is fearless, in fact, according to H. L. Mencken, â€Å"The one permanent emotion of the inferior man is fear fear of the unknown, the complex, the inexplicable. What he wants above everything else is safety† (H. L. Mencken). In agreement to Mencken, nothing in this world is safe; we waste seconds; minutes; hours; even days worrying about our fears. Worrying about our fears of the unknown. I believe we can’t relate to this unknown universe, however, we can live with it. Instead of waking up each day wondering what the unknown is, we need to wake up everyday focusing on what we do know, and interpreting that information into what we understand. In order to find the meaning in our lives, we each need control over our lives. I control my life one hundred percent. I make me own decisions, choose my individual actions, and control my responses. We have complete control over our own lives. However, the one think we do not have control over is other people’s actions and decisions. Although we have the power in our individual lives, others may influence our judgments, choices, and conclusions. All of these factors affect whom we are, along with the fact that we can’t control others reactions, which may cause poor influence on us. For example, in the novel, The Odyssey, Odysseus sails a long journey home, facing many distractions, challenges, and adventures. He encounters many different monsters that stop him in his path, including Cyclops, The Sirens, and Poseidon. Although Odysseus can control his actions, and the way he fights, he has a weakness when it comes to his opponents. Just like Odysseus, we can control our actions and responses, but when it comes to having others involves, we have no control over how they can act and make their choices. Living life freely, with no regrets, and accomplishing all of our goals. That is a satisfying life. Taking risks, without worrying, loving ourselves, being happy with our achievements, and experiencing all different emotions is what life is all about. A variety of different feelings occur throughout our lives; we need happiness, excitement, fears, anger, and even tears to lead a fulfilling life. In the independent reading novel, entitled Speak; Melinda Sordino writes about emotions and feelings, â€Å"When people don’t express themselves, they die one piece at a time† (Anderson 58). Melinda believes the reasons people need to express themselves is because everyone needs to experience all kinds of emotions in order to interact and stay alive. In the Odyssey, Both Odysseus and Telemachus are involved by an overwhelming amount of emotions. Without the emphasis of their feelings without the novel, the story would have much less meaning and would not be as dramatic. Emotions add to our everyday conversations and feelings towards each other. We need them to live a satisfying, enjoyable life. I believe, in order to find the meaning of my life, I need to sit back, relax, and let the question answer itself. In order to survive, and live for as long as possible, I have to enjoy my life in the present. Not be thinking about the future, and where life is going to take me; but live life as it is brought to me, rather then bringing myself to my life. By being patient and letting my life come to me as time goes on, I am sure to survive for a longer, happier, and more exciting period of time. Yet, at the conclusion to each person’s life, the certainty of death must strike. Getting ourselves ready for this unpredictable day is very easy, as long as we’ve accomplished everything that we have set out to achieve, death may appear whenever it feels the need to. It is a misfortunate thing, though, as some deaths develop so quickly and suddenly, we do not have time to say our goodbyes. Nor do the victims have time to achieve their goals, and pass on wishes. An example of a sudden death is in the novel entitles Lost for Words, by Alice Kuipers. In the story, Sophie’s older sister died traumatically in a sudden train bombing. Due to the death, it ripped Sophie’s family apart. They stopped talking to each other, and everything became awkward between her and her mother. Because of the unpredictable death at such a young age, her sister was, sorrowfully, not ready for her time to come. She may have not achieved her goals in life, and got to take as many risks as life leads her too, making it an even bigger tragedy for Sophie and her family. There were a number of dreadful deaths in the Odyssey as well. These included Odysseus’ crew, Odysseus’ mother, and many suitors. From reading all of these disastrous scenes and stories about death, I have learned that in order to reconcile myself for the inevitability of death, I need to start living my life with many risks, and no regrets. By living my life without any doubts, and enjoying my life in the present, I can be sure to live a long, satisfying life, ready for the fate of death for whenever it decides to doom on me. I believe the meaning of life, is to find meaning in our own, individual lives. By waiting for life to come to me, and enjoying the present in a way of happiness, enjoyment, and risks, I can be sure to eventually come to a full understanding of why I am here, living today. Each life has a world of its own, and each person his own life. Each person in this world will have a different story to share, and a different meaning and impact on our world today.

Asthma is a common and chronic inflammatory disorder of the airways, associated with marked health and economic consequences. It is estimated that approximately 5.2 million people in the United Kingdom (UK) suffer from asthma, making the condition the most common long term illness in this country. Asthma accounts for 1,400 deaths per annum, with a third of these being among individuals under 65 years of age. Similarly, asthma also accounts for about 69,000 hospital admissions a year. It is estimated that more than half of the 5.2 million people with asthma in the UK do not have adequate symptom control. While 500,000 of these have asthma that is difficult to control with available medication, and are thought to be resistant to corticosteroids, asthma is not well controlled in approximately 2.1 million people for reasons s uch as non-concordance with medication (Asthma UK, 2004, pp3-7). As already mentioned asthma is a long-term chronic condition (LTC) and although chronic illness is not a newly-recognised phenomenon, the incidence and prevalence has significantly increased over the second half of the 20th century and continues to rise in the 21st century (Howie, 2005, p318). This is not least because of the aging population and advances in medical science diminishing the impact of infectious diseases. In addition, the emergence of unhealthy lifestyles is arguably the trigger for many non-communicable diseases such as chronic heart disease, type 2 diabetes mellitus, and chronic obstructive pulmonary disease to which asthma has similar pathophysiology. (Nissinen et al, 2001, p963). It is estimated that in the UK over 17.5 million people are affected by a LTC and 8.8 million have long term illness that severely limits their day to day ability to cope. It is proposed that those aged over 65 affected by a LTC are more likely to have multiple long term conditions, which makes care particularly complex. Debatably, unhealthy lifestyles and an aging population are the reasons for the high prevalence of LTCs contributing towards the large financial costs in caring for these patients who occupy up to 42 percent of all acute hospital bed days (Department of Health (DH), 2005b, p10). Arguably, these issues indicate that these patients are not receiving the care in ways that meet their needs or the needs of the health system. It is proposed that this is why management of LTCs is of specific importance to current Government health strategies. To this effect, numerous policy documents have been published that define the present philosophies for the management of LTCs. One particular policy; The National Service Framework (NSF) for Long Term Conditions was published by the DH in March 2005 (DH, 2005b, p24). The NSF predominantly concentrates on individuals’ with long-term neurological conditions. However, it is anticipated that to a great extent the guidance presented could be relevant to all long-term conditions. The document has outlined 11 â€Å"quality requirements† and among others there are various ones that are particularly pertinent to Steven Williams’s case. These include the provision of person-centred care and choice, the offering of information and support for the safe and effective use of medicines, the supporting of self care and the consideration of health promotion needs. It is suggested that in order to meet Steven’s needs he will require support and education so that he can make informed choices. Metcalf (2005, p60) suggests that informed choice for those with LTCs is the key to success and a means of examining issues pertaining to non-concordance, risk taking behaviours and patient choice. As already mentioned, Steven began to smoke when he started attending university. Cigarette smoking is implicated as a health-risk behaviour and there is evidence to suggest that active smoking in adults with asthma increases asthma severity. A study by Siroux et al, (2000, p470) on the relationships of active smoking to asthma and asthma severity, found that current smokers with asthma had more asthma symptoms, more frequent asthma attacks (≠¥ 1 attack per day) and scored higher on the asthma severity scores, compared to those asthma sufferers who had never smoked and ex-smokers. Other trigger factors that can exacerbate asthma symptoms include house dust mites, pet allergens, pollen, moulds and fungal spores, certain drugs such as asprin and beta blockers, occupational triggers and viral respiratory tract infections (Roberts, 2002, p46). Arguably, in Steven’s case, alongside his lack of concordance with his asthma medication, it is suggested that his smoking habit is a key factor in his acute asthma exacerbations. Therefore, it is proposed that Steven needs help with smoking cessation and education on the side effects and concordance of his medication. Numerous approaches are presently being utilised for smoking cessation. These approaches incorporate pharmacological methods, such as nicotine replacement therapy or antidepressants, hypnotherapy, and exercise supported interventions. Behavioural approaches include stage based interventions, which mainly use the transtheoretical model (Prochaska, DiClemente Norcross, 1992, p1102-14) and this model divides people into five different stages. These are the precontemplation, contemplation, preparation, action, and maintenance stages. The justification behind â€Å"staging† people, as such, is to fit the therapy to a person’s need at his or her particular point in the change process. Succession through the stages is in order, although relapses to previous stages can happen. The model also recognises 10 processes of change, the theory being that the effectiveness of the different processes of change will vary according to the patient’s stage. Arguably, however, this h as not repeatedly been defended in empirical research (Sutton, 2000, p31). It is proposed that it would be necessary for health professionals to recognise precisely an individual’s stage of change, or readiness to change. This is so that an intervention based on â€Å"stage specific processes† of change can be employed. It is important that the stage of change is re-evaluated regularly, and that the intervention should reflect changes in the individual’s willingness to change. These elements of the intervention can be continual until the person accomplishes and sustains the change in behaviour. In this way, stage based interventions develop and adjust in answer to the individual’s progression through the stages of change. Therefore it is debated that stage based models recommend that interventions that take into account the existing stage of the individual will be much more successful and efficient than one size fits all interventions (Prochaska, DiClemente Norcross, 1992, p1103). Having said this however, the stages of change t heory does not take into account any outside influences that might have an impact on a person’s ability to change. It is proposed that Steven recognises that he has a problem and has asked for help. Therefore, it is suggested that this places him in the contemplation stage. It is suggested therefore, that Steven needs to be given help and advice that will lead him to the preparation for action stage. In doing this, debatably, it will be necessary for Steven to assess his feelings regarding his smoking behaviour. It is important therefore that health professionals who are using behavioural change models for smoking cessation are thoroughly trained in the procedure or at least are aware of the availability of a smoking cessation nurse. Ethically, it is argued that health professionals have a duty of care to help patients like Steven live healthier lifestyles. However, ethically Steven has the right to autonomy in his lifestyle choices (Tschudin, 2003, p151). It is proposed that inhaled corticosteroids are still the most effective preventer drug for attaining treatment objectives (British Thoracic Society, Scottish Intercollegiate Guidelines Network (BTS, SIGN, 2004, Chapter 4, p2). Steven has voiced concerns about the effects of steroids and this has stopped him taking his preventative inhaler. Similarly, he only uses his reliever inhaler when he becomes extremely wheezy. This is in accordance with Bender’s (2002, p554) suggestion that one of the reasons people do not take their medication is because they are worried about side effects. It is argued that this could be because their initial concerns might have not been fully addressed by health professionals (Carter et al, 2003, p27). It is proposed that nurses are ideally placed to educate patients on the benefits of medication concordance. It is important that a nurse thoroughly explains the necessity of the treatment and any subsequent side effects. Inhaled corticosteroids are t he main preventative treatment for asthma sufferers. When taken twice daily at a low dose, corticosteroids are highly effective in reducing asthmatic symptoms, improving lung function, and reducing cellular inflammation. Systemic effects are rare on a low dose and most asthma patients are extremely well controlled on a low dose inhaler. Adverse local effects can include dysphonia and oral candidiasis. These symptoms can be relieved by either gargling or rinsing the mouth with water after inhalation (Roberts, 2002, p48). It is proposed that if Steven regularly takes his preventative inhaler then his asthma will be better controlled and he is much less likely to need systemic corticosteroids that can have adverse side effects such as weight gain and thinning of the skin when taken long-term. The NSF quality requirements of person-centred care and choice, and the offering of advice on the use of medication are relevant to the case mentioned, as is the consideration of health promotion needs. The patient will require help in giving up smoking as this is a major factor contributing to his repeat admissions to hospital. Help in the correct use of his medication is also required if he is to remain free from episodes of acute asthma. Nurses caring for patients like Steven will need to know what help is available with smoking cessation and the various options that can be offered to individuals who want to cease smoking. This quotation sums the knowledge a person would gather after making a vast study of different societies. The nature of humanity is being discussed, rather than physical size.    The Lilliputians are narrow-minded people who become angry over trivial matters, while the Brobdingnagians are a deeper people, in contrast. From an outsider's view, one nation would not be so great if another were not so poor. Actions which seem natural today may appear barbaric to the generations of the future just as the actions of the past generations abhor the students of today. Only by comparison to something better does a system of power or a way of life seem small or evil. This is the purpose of satire. When studying history, students may find the customs of the past primitive. However, the people of that time most likely did not believe that they were. Perspective is vital in any comparison. A person from a highly technological society cannot easily understand the lifestyle of a civilization n... ...person who sees it. The Cayman Islands :: essays research papers

Part I - The Question   Ã‚  Ã‚  Ã‚  Ã‚  When I was 18, my family took a trip to Cancun for Spring Break. I absolutely loved the beaches and palm trees, the white sand, hot sun and blue waves. Ever since then I have had a real passion for tropical islands. My favorite and perhaps the most beautiful Islands I’ve researched are the Cayman Islands. I’ve only seen pictures and heard stories but I’ve already decided that’s where I am getting married. I still however, don’t know anything about the islands. My love for these islands and my thirst for knowledge on the topic has driven me to do more research and find out the history behind the Cayman Islands. I want to know everything about them, so far the only thing I know is that there are three islands, Grand Cayman, Cayman Brac, and Little cayman. I would like to know more. I am hoping to learn about the early development of the islands, the people, growth, tropical climate and environment. I am doing this purely for my own reference and curiosity. Part II - Research   Ã‚  Ã‚  Ã‚  Ã‚  I began researching for this paper online because it was easy for me to use and a lot faster than looking up information in text books, and it’s a lot more accurate and up-to-date. Immediately I found about a hundred web sites worth printing. After scrolling through page after page, site after site I realized I had barely scratched the surface, so I decided to pick the ones with the most promising titles and print them all. Then I began the tedious task of sifting through all those papers until I found the best ones. I think I ended up with about six different sites, all on different things about the islands. One of the best ones was about the history of the Cayman Islands, I found this to be the most factual. Another good one mostly was about the Geography and Geology of the islands. The rest of them were mainly about visiting the islands, the discovery of the islands, touring the islands and comparing and contrasting the three islands.   Ã‚  Ã‚  Ã‚  Ã‚  After this I went to the Jackson Community College Learning Center and read through books and Encyclopedias about Cayman Islands but I found most of these to have the same information as my web sites, but not as good. I found a few pictures and maps that were helpful in my own understanding of the islands, unfortunately they wouldn’t be much help with my paper. Do you agree with this interpretation of the importance of the battle of the Somme? Essay

Source A is an extract from a report written by General Haig in December 1916 about a month after the battle of the Somme, the report was then sent to the British cabinet. Douglas Haig was commander in chief of the British army and in the report he was trying to show how vital he thinks the battle was in perspective of winning the war. As with every source, it needs to be looked at in context so you can review its provenance and reliability. When the report was writhen there was a new government in Britain. David Lloyd George who had been Secretary of State for War was now Prime Minister replacing. Haig had been criticised by Lloyd George, Churchill and Beaverbrook and for his strategy of attrition at the Somme, this being the cause of huge casualties during the battle and only a very small push in land taken. The British only gained an average of 5miles for 420,000 casualties. Haig was now starting his own battle back home to save his reputation as a military commander.In the report Haig seems to have been using the report to justify his decisions that had led to the largest loss of forces and manpower ever experienced by the British army in one battle. Haig was commander in chief during the Somme and it was his job to set the tactics for commanders to carry out. The fact that Haig was Commander in Chief at the Somme meant that he was largely responsible for the tactics used at the battle. This means that in source A Haig is having to defend or justify his actions in on the Somme. This will make the source less reliable as Haig has cause to be biased in his report. He is trying to keep his job and wants to be known as a great military hero not a military disaster. This would have given Haig even more motive to give a biased report. Haig was respected by the French generals and had the support of the king, this put him in a position of power particularly as there was no one better to do the job. He knew that members of the cabinet did not like him being Commander in Chief so this may have made Haig present the evidence in source A in a more positive light. He did this to protect his reputation, justify his decisions and to keep his job. There is no evidence that Haig was a untruthful man. However, the likelihood that he was badly informed about the situation and his self-confidence and the fact that he was wanting to protect his reputation and job mean that Haig could be biased in his report and his reliability as a source can be doubted. Looking at Source A, it looks like most of the extract is accurate but not completely accurate. Haig states, ‘the German casualties have been greater than ours ’. This is misleading as although the British casualties were less than the German casualties (420,000-500,000), the combined allied casualties were more. The allies suffered 620,000 whereas the Germans suffered about half a million casualties. Nevertheless Haig may not have known about this at the time. Roll calls at the end of battles meant there were truthful records of allied casualties however German records would not have been available, so these casualty figures were estimates. Nonetheless Haig may have said the German casualties were greater than ours because he wants to keep his job and make his situation look better than it is. Haig admits that, ‘the amount of ground we have gained is not great’ which is true but he then says, ’That is nothing’. Even before the Somme, Haig had been more concerned with a strategy of attrition so. Haig was expecting his strategy to have large casualties but the public wanted a justification of why so many lives had been lost. Even though the German army had been disconnected from ‘strong defensive positions’ the cost was very high and the Germans had often retreated to equally strong defensive positions. Haig was presenting only part of the situation. He is doing this to put himself in a positive light. Because he wants to keep his job and be seen as a public hero rather than villain. However writing about the Somme in 1919 the German General Ludendorff wrote; ‘We had heavy losses in men and material. As a result of the Somme we were completely exhausted on the western front.’ This German quote supports Haig’s opinion of the effect of the Somme on the troops in the German army. Other than reports from captured prisoners its unclear how he can have been sure of this at the time. Whilst he has not lied in the report, Haig might be seen as guilty of laying out the information in a way that isn’t completely truthful. Also he has been discriminatory by only mentioning the upbeat information. Perhaps the principal thing wrong with the extract is it not showing the impact of the Somme on the British troops, but this would have undermined his argument because this would have showed the down side of the war and therefore not put Haig in a positive light. In conclusion, it appears that in the extract Haig is justifying his strategy of attrition at the Somme by showing the significance of the battle, for demonstrating the might of the British army, and its role in the destruction of the German army. Haig is reasonable in his view. It seems from the Reich archive that the affect on the German army from the Somme was great but nevertheless they still fought on for 2years. There are a number of key reasons why the Somme was important. It helped the French defence of Verdun and allied relations. It was a trial for new technologies in warfare such as tanks, machine guns and planes. Before the Somme, Kitchener’s volunteer army had been amateurs and inexperienced. After the Somme, they were a force of battle hardened experienced troops. The objectives that were achieved through the Somme were done through a strategy that cost many lives and unfortunately many errors made the battle longer. There is no way of telling whether the same things could have been achieved using different tactics. Haig was writing his report to people who were hostile to him and his strategy. He believed in his strategy and his ability to win the war, he was also worried about defending his reputation and job. His original aim was to wear down the German army in a war of attrition. From his view he achieved his aim so it is understandable why he wrote what he did. It is my view that what he said he achieved was important but even as an report to the British government themselves it presents a biased view because Haig has all the motive to be biased.
